29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded:    a) Rear side, new townhome construction located at 119 Alderwood Hill, Sandy Springs, GA 30328: Employee operating a DeWalt (Model-D28715) 14" Heavy Duty Chop Saw with 14" Diamond Blade was exposed to amputation hazards while using this equipment to cut bricks without the lower guard in operation. A cardboard shim was placed in between both guards to keep the lower guard from retracting after a cut was made.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:    a) Rear side, new townhome construction located at 119 Alderwood Hill, Sandy Springs, GA 30328: Employees were exposed to falls of approximately 15-feet, 4-inches while accessing multiple scaffold platforms using a single scaffold plank which measured 10-inches wide. Employees were not provided with a 18-inch or greater walkway while accessing the different scaffold sections.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces and scaffold frames were used for access.    a) Rear side, new townhome construction located at 119 Alderwood Hill, Sandy Springs, GA 30328: Employees were exposed to fall hazards of approximately 15-feet, 4-inches while working on a tubular welded (masonry walk-thru style) scaffold which was not provided with a ladder for safe access. Employees accessed the work platform by climbing the frames and the cross braces.
