29 CFR 1910.23(a)(6): Every manhole floor opening shall be guarded by a standard manhole cover which need not be hinged in place. While the cover is not in place, the manhole opening shall be constantly attended by someone or shall be protected by removable standard railings.    EXTRUSION DEPARTMENT: The employees working around an inadequately guarded open floor hole to install a new extrusion line were exposed to a seven foot fall into the open hole.    On or about 1/28/2014, while clearing debris and equipment out of the way of a forklift carrying a large piece of equipment, the employee picked up an unsecured, unmarked 26 inch spool reel that had been placed on top of an manhole opening and fell into the hole sustaining multiple injuries during the fall.

29 CFR 1910.145(f)(3): Use. Tags shall be used as a means to prevent accidental injury or illness to employees who are exposed to hazardous or potentially hazardous conditions, equipment or operations which are out of the ordinary, unexpected or not readily apparent. Tags shall be used until such time as the identified hazard is eliminated or the hazardous operation is completed. Tags need not be used where signs, guarding or other positive means of protection are being used.
