29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about February 5, 2014, at the above addressed site, an employee performing sign installation work was exposed to a fall hazard of approximately 13 feet 6 inches while in the basket of a truck mounted aerial lift and not wearing a harness with a lanyard attached to the boom or basket of that lift.
