29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:    (a) On or about 01/27/14 - 4205 Cogswell Avenue, Pell City, AL, an employee working from a Genie GS-3384 scissor lift was standing on the mid and upper guardrail.  The employee was exposed to a 14-foot fall.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) On or about 01/27/14 - 4205 Cogswell Avenue, Pell City, AL, an employee working from a Genie GS-3384 scissor lift was standing on the mid and upper guardrail. The employee was not trained on the hazards of working on this type of scaffold.
