29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high:   a.  On or about February 5, 2014, on the west side of the building, the scaffold platforms did not have midrails, exposing employees to a 10 1/2 to 14 feet fall hazard.

The Keith Harris Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 315481275, citation number 01, item number 002, and was affirmed as a final order on January 30, 2012, with respect to a workplace located at 4594 N Plalfox Street, Pensacola, FL.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a.  On or about February 5, 2014, on the west side of the building, the scaffold platform did not have a ladder or other safe means of access, exposing employees to a 14 feet fall hazard.
