29 CFR 1926.25(a):  During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) On or about January 30, 2014, at worksite 19301 E 50 Terr Independence Missouri. Employees were exposed to triping hazard when ladders not in use were stored in the work areas and passage ways.  (b) b) Employees were exposed to tripping hazards when 2 X 4's, electrical cords, air hoses and spent round casings from a powder actuated tool were laying in work areas and passasge ways.

29 CFR 1926.150(c)(1)(i):   A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof:  (a) On or about January 30, 2014 at worksite 19301 E 50 Terr Independence Missouri. Employees were exposed to a fire hazard when working in a 38,000 square feet building with 3 working fire extinguishers in the protected building areas.
