29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)Bible Baptist Church, 7975 Spanish Fort Blvd. Spanish Fort, AL:  Employer failed to ensure employees used guardrails, safety net or fall arrest system  to prevent falls of 9 feet to a grass surface while performing roof shingling  job tasks on a roof with a pitch of 6/12 and accessing the roof with a ladder that was not adjusted 3 foot over roofline.
