29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket:  a.  On or about February 6, 2014, on the north side of the building, an employee working from an aerial lift was climbing the rails of the aerial lift, exposing themselves to a 18 feet fall hazard.  b.  On or about February 6, 2014, on the south side of the building, an employee working from an aerial lift was climbing the rails of the aerial lift, exposing themselves to a 18 feet fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a.  On or about February 6, 2014, on the North side of the construction site, an employee using an aerial lift, was not wearing harnesses and lanyard attached to the boom or basket, exposing himself to a 18 feet fall hazard.  b.  On or about February 6, 2014, on the Sorth side of the construction site, an employee using an aerial lift, was not wearing harnesses and lanyard attached to the boom or basket, exposing himself to a 18 feet fall hazard.
