29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  a) At the site, where the 100 foot flexible cord was wedged between the door and the door frame of a newly constructed building.  This condition exposed employees to an electrical hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) At the site, where employees used an extension ladder that was not extended at least three feet above the landing surface to access the roof structure of a newly constructed Chick-fil-a building.  This condition exposed the employees to a fall hazard.
