29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder.           On or about February 5, 2014, workers were engaged in construction activities installing waste water treatment updates to Georgetown, Ohio's existing waste-water treatment facility. A worker was working on the upper section of a Werner,  40.0 foot, orange, fiber glass, extension ladder, exiting a 29.0 foot x 8.0 foot round, concrete pump station. The worker was exposed to potential 29.0 foot fall hazards in that the ladder was not properly protected from accidental displacement from a blow out of the pressure testing plug.
