29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses:   A)    GrowCo., Inc., located at 844 Kochheiser Road, Mansfield, OH 44904: On or about January 23, 2014, the employer failed to guard the point of operation of the Komatsu 110 mechanical power press (Model OBS110S-VS-3, Machine # D0028, 110 Ton) located in the GrowCo building. Employees were exposed to amputation hazards while operating the press.  No additional abatement information is needed for this item.

29 CFR 1910.217(d)(9)(i): The employer did not establish a die setting procedure for mechanical power presses that would ensure compliance with 29 CFR 1910.217(c):   A)    GrowCo., Inc., located at 844 Kochheiser Road, Mansfield, OH 44904: On or about January 23, 2014, the employer did not establish die setting procedures for the Komatsu 110 mechanical power press (Model OBS110S-VS-3, Machine # D0028, 110 Ton), located in the GrowCo building, to ensure that employees were protected from amputation hazards created by the point of operation.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power presses was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment:   A)    GrowCo., Inc., located at 844 Kochheiser Road, Mansfield, OH 44904: On or about January 23, 2014, the employer failed to establish and follow a program of periodic and regular inspections for the Komatsu 110 mechanical power press (Model OBS110S-VS-3, Machine # D0028, 110 Ton), located in the GrowCo building, to ensure that all parts, auxiliary equipment, and safeguards of the press were in a safe operating condition and adjustment. The press had deficiencies, exposing employees to amputation hazards when operating the press.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.217(f)(2): Operator(s) were not trained and instructed in the safe method of work before starting work on mechanical power presses:   A)    GrowCo., Inc., located at 844 Kochheiser Road, Mansfield, OH 44904: On or about January 23, 2014, the employer did not ensure that employees were trained in the safe operation of the Komatsu 110 mechanical power press (Model OBS110S-VS-3, Machine # D0028, 110 Ton) located in the GrowCo building. Training was not provided regarding the hazards associated with control of mode selection. Operators were exposed to amputation hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
