29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):  (a) On or about 2/7/14 - 751 Montclair Road, Birmingham, AL, the platform of a ladder jack scaffold was not extended at least 6 inches or secured to prevent it from slipping and employees were exposed to falls of 12 feet to the ground below.
