29 CFR 1910.29(a)(3)(v): The work level of scaffolds (towers) did not extend for the full width of the scaffold:      On or about February 5, 2014, at 5847 Hwy 1 South, Jonesboro, AR, the working level of the scaffold consisted of three boards approximately 12 inches in width with plywood placed over the boards.   The plywood did not extend the full width of the scaffold.     This condition exposed employees standing on the work platform to the hazard of stepping into the gaps between the plywood and the end frames of the scaffold and falling to the concrete floor below, approximately 11 feet.

29 CFR 1910.29(a)(3)(vii): Work levels 10 feet or higher above the ground or floor did not have  a guardrail of 2" x 4" nominal or the equivalent, installed no less than 36" or more than 42" high, with a mid-rail of 1"x 4" nominal lumber or equivalent:   On or about February 5, 2014, at 5847 Hwy 1 South, Jonesboro, AR, the working platform on the manually propelled mobile scaffold was not equipped with guardrails.  This condition exposed employees standing on the work platform to the hazard of falls to the concrete floor below, approximately 11 feet.
