The plug on the power cord of the fan lacked a ground prong. NO ABATEMENT CERTIFICATION REQUIRED

The outer sheath of the blue extension cord was badly frayed and crushed in spots, exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
