29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 4 February 2014, on the above addressed jobsite, employees were utilizing a pneumatic nail gun to install plywood unto the concrete wall on the 2nd floor of the residential home under construction, without the use of eye protection, exposing the employee to the hazard of being struck by flying projectiles or debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 4 February 2014, at that above addressed jobsite, two employees were installing plywood on the 2nd floor of the home under construction, without the use of fall protection, exposing the employees to a fall hazard of approximately 24 feet in height to the ground below.
