At the time of the inspection, the green Clarke propane fork truck was being operated while in disrepair. a) The horn on the forktruck was inoperable. b) The back up alarm on the fork truck did not work.

At the time of the inspection, at least two (2) operators of the green Clarke propane fork truck, did not receive a combination of formal instruction, practical training and exercises, and evaluation on fork truck operation.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. 1. Inside of the repair shop, near the north bay doors, employees were using a duplex metal outlet box with knockouts as an extension cord to energize tools and equipment. 2. Inside of the repair shop, near the south bay doors, employees were using a duplex outlet box with knockouts as an extension cord to energize tools and equipment. 3. Inside of the repair shop, near the north bay doors, employees were using a power tap that was mounted to shelving and plugged into a duplex metal outlet box with knockouts as an extension cord to energize tools and equipment.
