29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6m) above or below a point of access the employer did not provide one of the following; portable ladders, hookon ladders, attachable ladders, stair towers (scaffold stairways/towers), stairwaytype ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold. Crossbraces shall not be used as a means of access: a) The employer did not ensure that employees use safe access to scaffolds (scizzorlift). The employee stepped from the revolving door roof onto the top of the scizzorlift railings; was exposed to 7 ft fall hazard. The lift was not positioned so the railing access gate was accessible to the employee. Location: 475 5th Ave, New York, N.Y. On or about 2/10/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

The employee stepped from the revolving door roof onto the top of the scizzorlift railings; was exposed to 7 ft fall hazard.
