29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  JOBSITE:  The employee, standing on a stone/concrete block wall to monitor the flow of water from a pond and guide an excavator operator, was not protected from the hazard of falling approximately 13 feet to the ground below by the use of guardrails, personal fall arrest system or other equivalent means.
