29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  At jobsite located at 1842 W. 28th St, Lawrence, KS, employees used a 100 feet long extension cord and electrical jigsaw that was not protected by a ground fault circuit interrupter in that the wiring was tapped off an exterior light located under the eave of the garage entrance, exposing employees to an electrical shock hazard.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At jobsite located at 1842 W. 28th St, Lawrence, KS, the path to ground for the extension cord used to power a saw being used to cut siding was not continuous in that the ground prong was missing from the male end of the cord connected to the source, exposing employees to a shock hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  At jobsite located at 1842 W. 28th St, Lawrence, KS, a pumpjack scaffold used to facilitate siding installation rested on the wet ground directly on the baseplates without mud sills, exposing employees to a fall hazard of approximately 8 feet to the ground below.
