On or about February 12, 2014, employees were exposed to struck by hazards in that the employer did not evaluate each forklift operator's performance once every three years.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about February 13, 2014, employees were exposed to fall hazards in that the employer did not ensure that employees installing floor joists on the top of the building above 6 feet were protected from falling by a guardrail system, safety net system, or a personal fall arrest system.
