29 CFR 1926.403(i)(2): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:    a) North end of building, 1st floor corridor, on or about 2-13-14:  Employees were working in the vicinity of an energized junction box with multiple extensions, open in several locations, with live parts visible and accessible.     b) East side of building, entry/elevator area, on or about 2-13-14:  Employees were working in the vicinity of a 12" x 12" open top energized junction box with live wires protruding, capped only with wire nuts.     c) East side of building, entry/elevator area, on or about 2-13-14:  Employees were working in the vicinity of an energized junction box with gaps between box extenders with live parts visible and accessible.
