29 CFR 1910.269(l)(2): The employer did not ensure that no employee(s) approached or took conductive objects closer to exposed energized parts than set forth in Tables R-6 through R-10:  a) International Paper Mill, Prattville, AL, Pulp Mill number 2; on or about February 11, 2014 and times prior the employer failed to provide isolation beyond the twenty five inches Minimum Approach Distance (as per Table R-6) or insulation or isolation between the de-energized 500 MCM cable being cut and the adjacent 13.8 KV energized line.    In the alternative:  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer allowed employees demolishing de-energized cable within approximately five inches of adjacent energized 13.8 kV cable without any insulating or cut proof barrier between the cables. The employer failed to follow IEEE National Electrical Safety Code, C2-2012,  Section 443.A.4.  a) International Paper Mill, Prattville, AL, Pulp Mill number 2; on or about February 11, 2014 and times prior the employer failed to provide isolation beyond the twenty five inches Minimum Approach Distance (as per Table R-6) or insulation or isolation between the de-energized 500 MCM cable being cut and the adjacent 13.8 KV energized line.
