29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:   (a) On or about February 12, 2014, for the employee who had sustained a work-related injury on January 4, 2014, that had not been recorded on the OSHA Form 300 at White Senior Living located at 200 South Patrick Avenue in White, South Dakota.   (b) On or about February 12, 2014, for the employee who had sustained a work-related injury on January 6, 2014, that had not been recorded on the OSHA Form 300 at White Senior Living located at 200 South Patrick Avenue in White, South Dakota.
