29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a). Worksite located at 525 NW 64th Street, Kansas City, Missouri- Employees were exposed to overhead hazards while working and installing the sign and were not protected from impact by the use of any means of head protection.

Worksite located at 525 NW 64th Street, Kansas City, Missouri- Employees were observed working in aerial lifts without the use of any means of fall protection.
