(a)  At 12525 St. Frances St. S.:  On February 14, 2014 and at times prior to; the employer failed to require the use of eye protection while using the pneumatic air guns to affix asphalt shingles to the roof, exposing employees to struck-by injuries from nail or wood fragments.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  At 12525 St. Francis St. , Lillian, AL:  On February 14, 2014 and at times prior to; the employer faild to provide fall protection for employees affixing asphalt shingles to an 8:12 pitch roof exposing the employees to falls of approximately 11.6 feet.
