29 CFR 1910.37(a)(3): Exit routes were not continuously maintained free of obstructions or impediments to full instant use in the case of fire or other emergency: a) 256 Sheffield Street Building Northeast exit: The Exit route was not continuously maintained free of obstruction. The exit discharge was covered with snow preventing the emergency exit door from fully opening. b) 256 Sheffield Street Building Northwest exit: The Exit route was not continuously maintained free of obstruction. The exit discharge was covered with snow preventing the emergency exit door from fully opening. c) 256 Sheffield Street Building Northwest exit: The Exit route was not continuously maintained free of obstruction. Paper supplies used for the Folding Machine, wood pallets and a work table were obstructing the path to the Northwest exit door. Condition noted on/or about February 14, 2014.

29 CFR 1910.95(g)(1): An audiometric testing program was not established and maintained for all employees whose noise exposure equaled or exceeded an 8 hour time weighted average of 85 dBA: a) 256 Sheffield Street Building Binding Area: The Employer did not establish and maintained audiometric testing for employees exposed to noise level above 85 dBA. Employee working on the folding/glue machine (B&R Moll Pocket Folder Specialist) was exposed to continuous noise level of 87.68 dBA for an 8-hour TWA, which is equivalent to a dose of 72.48%. The exposure exceeds the Action level of 85 DBA. Sampling time was 443 minutes. Zero exposures were assumed for all un-sampled time periods. Violation occurred on/or about May 21, 2014.

Training shall consist of a combination of formal instruction, practical training, and evaluation of the operators performance in the workplace. Condition noted on/or about February 14, 2014.
