29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: (a)CH Roofing, LLC @170 S Jackson Street, Denver, CO 80209: On and before February 14th, 2014, the controlling employer did not ensure that subcontractor employees were working on a residential roof with proper fall protection. Two subcontractor employees were tearing off roofing material near the edge of the roof on a multi-family apartment complex without being tied-off. This condition exposed employees to fall hazards of approximately 20 feet.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. (a)CH Roofing, LLC @ 170 S Jackson Street, Denver, CO 80209: On and before February 14th, 2014, the controlling employer did not ensure that subcontractor employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns were protected by hard hats. A subcontractor employee working on the ground was not wearing a hard hat while exposed to falling and flying debris from the roof. This condition exposed employees to struck-by hazards.
