29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a)Total Excavation Services at 1107 12th Street, Boulder, CO 80302: On and before February 13th, 2014, the employer did not ensure that the excavation was protected by an adequate protective system along the north wall (9 foot high vertical wall) and east wall (6 foot high with a slope greater than 66 degrees) in Type B soil. The condition exposed employees to cave-in hazards.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: (a)Total Excavation Services at 1107 12th Street, Boulder, CO 80302: On and before February 13th, 2014, the employer did not ensure that a competent person inspected this excavation along the north and east walls prior to the start of work and as needed throughout the shift. The condition exposed employees to cave-in hazards.
