OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards. On or about February 18, 2014, at the work site located at Energy Service Company, Rig# 83, 1300 Eagle Park Way, Fort Worth, Texas: The Auxiliary Escape line anchor point was not properly installed prior to the start of operations.

29 CFR 1910.179(h)(2)(v): Rope clips attached with U-bolts did not have the U-bolts on the dead or short end of the rope. On or about February 18, 2014, at the work site located at Energy Service Company, Rig# 83, 1300 Eagle Park Way, Fort Worth, Texas: The employer did not ensure the auxiliary escape line cable clamps were properly installed to the Auxiliary cable exposing employees to a fall hazard.
