On or about February 18, 2014, employee(s) were observed working from the Genie S-65 aerial lift without the use of a body harness and lanyard and were exposed to a fall hazard of approximately 20 feet.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   Work-site located at 2831 Conference Drive. Norman, OK 73069. On or about February 18, 2014, employees were observed installing metal trim to the exterior of the building from the Genie S-65 aerial lift and were not standing on the floor of the work basket.  Employees were exposed to a fall of approximately 20 feet.

On or about February 18, 2014, an employee was observed working below an aerial lift without a protective helmet and was exposed to falling objects from metal work being performed overhead.
