29 CFR 1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation:    a)  On or about March 6, 2014, and at times prior, the injury and illness log (OSHA form 300) for 2014 did not contain accurate information for all injuries and illnesses which occurred in 2014.  An allergic reaction which was reported on February 11, 2014 did not have the correct name of the employee.  b)  On or about March 6, 2014, and at times prior, the injury and illness logs (OSHA form 300) for 2011 and 2013 had entries that did not specify the facility in which the injury occurred.

29 CFR 1904.30(a): A separate OSHA 300 Log was not maintained for each establishment that was expected to be in operation for one year or longer.  a)  On or about March 6, 2014, and at times prior, the employer only maintained one injury and illness log (OSHA form 300) for multiple facilities for the years 2011 through 2014.
