29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A) Martin Sprocket & Gear, Inc., located at 350 South Airport Road, Montpelier, OH 43543: On or about February 13, 2014, the employer failed to utilize energy control procedures to control hazardous energy of the Newark gear hobbing machine D (Model P&J 7G, Serial # 701686). Employees did not control all hazardous energy sources such as, but not limited to, electrical, while performing activities such as, but not limited to, setting-up the machine and changing cutters. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: A) Martin Sprocket & Gear, Inc., located at 350 South Airport Road, Montpelier, OH 43543: On or about February 13, 2014, the employer did not ensure that the Newark gear hobbing machine D (Model P&J 7G, Serial # 701686) was properly turned off/shut down while employees conducted activities such as, but not limited to, setting-up the machine and changing cutters. Employees were exposed to injuries from unexpected start-up of the machine and energy release. modified to: A)&&.. the employer did not ensure that a lock was used to isolate the electrical energy on the Newark gear hopping machine D after it was shut down when setting-up the machine or changing the cutters.
