Berenfield Containers, Inc., 304 Main Avenue, Warren, Pa 16365, parts lining booth area, on or about February 14, 2014 and times prior thereto:  Employees perform painting/coating operations in an Excel brand parts lining spray paint booth.  Materials including, but not limited to, a worktable, metal containers containing used and unused rags, were stored within one foot of the parts lining spray booth.
