29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:   On or about February 20, 2014, at a commercial construction site, the employer did not ensure the electrical extension cord used to power a 7-1/4  inch 120V 12 Amp Craftsman Evolv Circular Saw Model 320.18780 SN 00563 was equipped with a ground pin exposing employees to electrical shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about February 20, 2014, two employees working on the rooftop of a commercial building performing air conditioning work installing a roof curb were not protected from fall hazards. The employees were exposed to fall hazards of more than (14) feet to the ground below.
