29 CFR 1910.178(p)(1): Powered industrial trucks found to be in need of repair, defective, or in any way unsafe, were not taken out of service until they had been restored to safe operating condition:  On or about February 19, 2014- Employees were exposed to serious struck by injuries from the Recyclifts Forklift that was in operation, in that the slip resistant brake pad was found to be missing, or defective and in need of repairs:  Abatement certification is required for this item.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   On or about February 19, 2014- Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the Pulley System on the sticher did not have a guard that fully enclosed the moving parts on all exposed sides.            Abatement certification is required for this item.

29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less.   On or about February 19, 2014-  Employees were exposed to electrical hazards in that there was not an adequate entrance to the electrical panel box as the entrance was only 13 inches wide as it was narrowed by a compressor and a cylinder box on the floor.  Abatement certification for this item is not required.
