29 CFR 1910.304(b)(3)(i): All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops did not have ground-fault circuit-interrupter protection for personnel On or about February 20, 2014, and times prior thereto, electrical outlets used to power whirlpool tubs in two whirlpool rooms did not have ground-fault circuit-interrupters installed. Employees were exposed to the hazard of electric shock.

29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place: On or about February 20, 2014, and times prior thereto, the employer did not ensure that titers were performed following the completion of hepatitis B vaccine serious as recommended by the U.S. Public Health Service. Employees were exposed to the hazard of contracting hepatitis B from exposure to contaminated blood.
