29 CFR 1910.272(m)(4): The employer did not implement procedures in the grain handling facility for the use of tags and locks which would prevent the inadvertent application of energy or motion to equipment being repaired, serviced, or adjusted, which could result in employee injury:    Specialty Grains, Inc. had employees perform maintenance on equipment within the facility and the employer did not implement procedures for the use of tags and locks to prevent inadvertent application of energy or motion to equipment being repaired, serviced or adjusted.  This would include:       a.  Drum Cleaner  employee removed guard to measure pulley and didnt lockout equipment (February 13, 2014) and  b.  Bin #3  two employees locked out conveyor and took lid off conveyor, but only one employee applied a lock to electrical disconnect (February 25, 2014).
