At the workplace, located in Don's area, there was a flexible cord used as permanent wiring to supply 110 volt power to an AB Dick printing press air pump, thereby exposing the employees to electrical shock and fire hazards. Carbonless and Cut Sheet Forms was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 282327, citation number 1, item number 7 and was affirmed as a final order on August 10, 2012, with respect to a workplace located at 1948 John Glenn Highway, New Concord. Ohio 43762.

At the workplace, employees were pouring blanket wash from 55 gallon drums into smaller containers that did not have labels indicating the contents, thereby potentially exposing the employees to hazardous chemicals. Carbonless and Cut Sheet Forms was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 282327, citation number 1, item number 9 and was affirmed as a final order on August 10, 2012, with respect to a workplace located at 1948 John Glenn Highway, New Concord.
