29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Construction site - On or about February 21, 2014, an employee using an angle grinder on a metal door frame did not wear protective eyewear to protect from metal chips, fragments and sparks. b) Construction site - On or about February 21, 2014, an employee using a DeWalt chop saw to cut metal did not wear protective eyewear to protect from metal chips, fragments and sparks.
