On or about February 10, 2014, at the above addressed jobsite, employees were using a job-made Personal Fall Arrest System (PFAS), while working/riding on a material hoist, for which the arresting component of the system was designed by the equipment manufacturer to be utilized as a tower-ladder climbing safety sleeve device and not a personal fall arrest system rope grab device, therefore exposing the employees to fall hazards of approximately 60 feet.
