29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about February 18, 2014, employees performing cornice installation on a residential home were working from a single pole job built scaffold systems and were exposed to an unprotected fall: a) at the front north west corner of the home, an employee was exposed to an unprotected fall approximately 14 feet to the ground below; and b) on the rear north west area of the home, an employee was exposed to an unprotected fall approximately 21 feet to the ground below.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about February 18, 2014, employees performing cornice installation on a residential home were working from single pole job built scaffold systems that did not have a platform at least 18 inches wide and were exposed to fall hazards: a) at the front north west corner of the home, an employee was exposed to falling approximately 14 feet to the ground below; and b) on the rear north west area of the home, an employee was exposed to falling approximately 21 feet to the ground below. The Mr. Jose Adan Alvarado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(2), which was contained in OSHA inspection number 316043645, citation number 1 item number 1 and was affirmed as a final order on 01/03/2012 with a final abatement date of 01/03/2012, with respect to a workplace located at 3521 Saratoga Downs, Fort Worth, Texas 76244.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access: On or about February 18, 2014, employees performing cornice installation on a residential home were working from single pole job built scaffold systems that did not have an accessible ladder for access and were exposed to a fall hazard: a) at the north west corner of the home, an employee was approximately 14 feet from the ground below; and b) on the rear north west upper mid-section area of the home, an employee was approximately 21 feet from the ground below. The Mr. Jose Adan Alvarado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 316043645, citation number 1 item number 3A and was affirmed as a final order on 01/03/2012 with a final abatement date of 01/03/2012, with respect to a workplace located at 3521 Saratoga Downs, Fort Worth, Texas 76244.
