29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about 21 February, construction site at 1295 McQueen Smith Rd, Prattville, Al: The employer failed to ensure employees working from an aerial lift (JLG model 400s, serial # 0300107087, unit #740-0006) at heights up to 16 feet while installing the Prolinx building wrap wore their fall protection which exposed them to the hazards associated with falls.
