29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about February 21, 2014, at the above address jobsite, the employer did not provide training to a concrete boom pump truck operator who was exposed to hazards such as but not limited to electrical shock from overhead power lines of 138 kV.

29 CFR 1926.600(a)(6)(ii): For lines rated over 50 kV, minimum clearance between the lines and any part of the crane or load were not 10 feet plus 0.4 inch for each 1 kV over 50 kV, or twice the length of the line insulator, but never less than 10 feet:    On or about February 21, 2014, at the above address jobsite, the employer had placed a concrete boom pump truck at less than the required 45.2 ft from the overhead 138kV power lines.
