29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     (a). Worksite located at 1540 E. Evergreen, Springfield, Missouri- Employees were exposed to possible eye injuries due to the stucco materials being applied to the walls and were not wearing any means of eye protection. On the day of the inspection, Feb. 20, 2014, wind gusts were stated to be in upwards or 40 to 50 mph.

Evergreen, Springfield, Missouri- Employees were exposed to a fall hazards in that end rails were not in place to prevent employees from falling. Employees were observed leaning out from the scaffold where the endrails should have been installed.

Worksite located at 1540 E. Evergreen, Springfield, Missouri- Employees were exposed to a fall and structural collapse hazard in that the scaffold's base and interior sections were missing cross-bracing and caused the scaffold to become plumb and square.

Worksite located at 1540 E. Evergreen, Springfield, Missouri- Employees were exposed to a fall or scaffold collapse hazard in that the scaffold sections were not secured together to prevent uplift. The scaffold was being used with wind gusts on the day of the inspection, Feb. 20, 2014, of up to 40-50 mph.
