29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a)Production Area: A Do-All bandsaw (serial number 290-722599) had a guard at the point of operation which exposed 2 inches of unused blade and another approximately 2 feet of exposed blade above that on or about 1/16/14.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  a)Production Area: Employees were exposed to electrical hazards from a 220 Volt electrical enclosure on the production side of the roll-up door wall and two 120V electrical enclosures on the storage side of the roll-up door wall which were not effectively closed due to missing circuit breakers on or about 1/16/14.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
