29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not shall establish and implement a written respiratory protection program with worksite-specific procedures:     This violation occurred on or about February 25, 2014 at Polytronix Inc. located at 705 N Plano Road in Richardson, Texas 75080; where at least three (3) employee were required to wear North half facepiece respirators for protection against acetone vapors.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse:    This violation ocurred on or about February 25, 2014 at Polytronix Inc. located at 705 N Plano Road in Richardson, Texas 75080; where employees worked around industrial steel storage racks that were not anchored to the ground and were not labeled with plaques informing operating personnel of the maximum permissable load of each rack level.
