29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   a)  On or about February 25, 2014 and at times prior to, employees were observed walking/working on the 2nd level of a new building under construction.  The employees were not protected from falling, exposing them to fall hazards from about 18 feet 8 inches to the lower level ground.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.     a)  On or about February 25, 2014 and at times prior to, an extension ladder was used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface or secured at the top to a rigid support, exposing the employees to fall hazard of about 10ft 4in.
