29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     The employer failed to implement a hazardous energy control program that includes but is not limited to: documenting specific energy control procedures, employee training and periodic inspections to ensure servicing and setup of production equipment is conducted safely.        The production equipment includes the following:   1.      Nexus QT 200B SN 201 958 Model QTN 200-II 2.      Nexus QT 200A SN 201 959 Model QTN 200-II 3.      Nexus QT 510C II SN 207-150 4.      Nexus QT 300 SN 227788 5.      Nexus QT 250 SN 215360  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Effective machine guarding was not provided for production equipment including the following:   1.      Nexus QT 200B SN 201 958 Model QTN 200-II: Interlocks not functioning.    2.      Nexus QT 200A SN 201 959 Model QTN 200-II: Interlocks not functioning.   3.      Nexus QT 510C II SN 207-150: No interlocks or safety devices.    4.      Nexus QT 300 SN 227788: No interlocks or safety devices.  5.      Nexus QT 250 SN 215360: No interlocks or safety devices.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
