29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) Roof, new residential construction located at 2660 Parks Edge, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a pneumatic Hitachi (Model NR83A2) Framing Nail Gun installing 1/2-inch OSB plywood sheathing with no safety glasses. The employee was firing 2-3/8-inch plastic collated fasteners at 140 PSI with no eye protection.  (b) First floor, new residential construction located at 2660 Parks Edge, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a pneumatic Hitachi (Model NR83A2) Framing Nail Gun installing headers with no safety glasses. the employee was firing 3-inch plastic collated fasteners at 140 PSI with no eye protection.  (c) First floor, new residential construction located at 2660 Parks Edge, Smyrna, GA 30080: Employee was exposed to eye injuries while operating a Makita (Model 5007F) 7-1/4-inch circular saw with no eye protection as he was cutting 2x4 and 2x10 header materials with no safety glasses.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a) First floor, new residential construction located at 2660 Parks Edge, Smyrna, GA 30080: Employee was exposed to severe laceration and amputation hazards while operating a Makita (Model 5007F) 7-1/4-inch Circular Saw without the lower guard in place. The saw was operated with a wood shim between the upper blade guard and the lower guard which prevented the guard from retracting after a cut was made.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a)  Roof, new residential construction located at 2660 Parks Edge, Smyrna, GA 30080: Employee was exposed to falls while working on a 6/12 pitched roof at eave heights of approximately 10-feet, 11-inches from the lower concrete surface with no fall protection system in place.
