29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.28(a), issued as citation 1, item 1, and described as follows:   a.  On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, the employer did not require the employees to wear appropriate personal protective equipment, such as a full body harness with shock absorbing "Y" lanyards while working on a 195 foot monopole communication tower thereby, exposing the employees to a fall hazard.  b.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.50(b), issued as citation 1, item 2, and described as follows:   On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, the employer did not make provisions for prompt medical attention in case of serious injury prior to the commencement of work.  c.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 17, 2014, for a violation of 29 CFR 1926.50(d)(1), issued as citation 1, item 3, and described as follows:   On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, employees did not have first aid supplies onsite, and were not easily accessible.  d.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.95(d)(1), issued as citation 1, item 4, and described as follows:   On February 25, 2014, at the construction site located at 43078 Elk Run, Coolville, Ohio, two employees were working on 195 foot monopole communication tower were required to purchase their own full body safety harness to be used for fall protection.  e.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.100(a), issued as citation 1, item 5, and described as follows:   On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, three employees were not protected from falling objects by protective helmets thereby, exposing employees to a head injury from impact.  f.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.104(d), issued as citation 1, item 6, and described as follows:     On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, an employee was wearing a full body harness with a single makeshift lanyard made from a Lift-All Webmaster 1200, Type EE2  601D general purpose lifting sling attached to the back D-ring for fall protection while climbing and working on a 195 foot monopole communication tower. The sling had a label which indicated a vertical capacity of 2,400 lbs., therefore not meeting the 5,400 pound nominal breaking strength.  On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, an employee was wearing a full body harness with a single makeshift lanyard made from a Lift-All Webmaster 1600, Type EE2  801 general purpose lifting sling attached to the back D-ring for fall protection while climbing and working on a 195 foot monopole communication tower. The sling had a label which indicated a vertical capacity of 3,200 lbs., therefore not meeting the 5,400 pound nominal breaking strength.  g.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 21, 2014, for a violation of 29 CFR 1926.405(b)(2), issued as citation 1, item 7, and described as follows:  On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, on the work truck, there was an electrical outlet, powered by a StatPower Prosine 1000 stand-alone inverter, that did not have a faceplate installed, exposing employees to an electrical hazard.  h.   The employer has failed to submit abatement certification and documentation within ten days of the abatement date, August 9, 2014, for a violation of 29 CFR 1926.503(a)(1), issued as citation 1, item 8, and described as follows:  On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, the employer did not provide a training program for employees exposed to fall hazards while climbing a communication tower.  i.   The employer has failed to submit abatement certification and documentation within ten days of the abatement date, July 17, 2014, for a violation of 29 CFR 1926.501(b)(15), issued as citation 2, item 1, and described as follows:  On February 25, 2014, at the communication tower construction site located at 43078 Elk Run, Coolville, Ohio, two employees were ascending and descending a 195 foot monopole communication tower without the use of fall protection thereby, exposing the employees to a fall hazard.
